Please familiarise yourself with some of the key information below to help us ensure highest levels of safety for all.

- Before you leave for the practice, have a meal and clean your teeth thoroughly afterwards
- Take your medications as normal
- Use the lavatory prior to departure
- With the exception of children and patients in need of support or supervision, please come alone
- We will provide hand sanitiser for you to use as necessary during your visit
- In the interests of public safety, please only attend your appointment if you are feeling physically well

- Our premises can support users up to 120kg/18.9st/264.5lbs

- Please contact the surgery to rebook if in the 14 days prior to your visit you have had:
o High temperature
o A new, continuous cough or sore throat
o Loss or change to your sense of smell or taste or runny nose

- If there have been any medical changes since your last visit, please let us know and a new Medical History Form will be emailed over for you to complete ahead of your appointment
